United States is eligible to be nominated by a teacher.
The application deadline is June 7, 2004.

Judges are appointed by calculus.org in consultation with members of the editorial board. If no prize is awarded for one or more years, two or more prizes may be awarded in subsequent years. The award of $1000 will be announced about July 1, 2004.
